I, the undersigned, declare that the information and supporting documentation provided in this Mouthful Foods Driver Application Form are true, accurate, complete, and up to date to the best of my knowledge.

I confirm that I am duly authorised to submit this application and that I am applying to become an independent Delivery Driver (Delivery Partner) on the Mouthful Foods Platform.

I acknowledge that the submission of this application does not guarantee approval or onboarding as a Mouthful Food Delivery Driver. Mouthful Foods reserves the right to approve, decline, request additional information, suspend, or withdraw this application at its sole discretion and in accordance with its internal policies and applicable law.

I further confirm that I hold, and will maintain throughout my engagement with Mouthful Foods, all valid licences, permits, registrations, certifications, insurance (where applicable), and other legal requirements necessary to lawfully operate my vehicle and perform delivery services in accordance with the laws of the Republic of South Africa.

By signing below, I confirm that I have read, understood, and agree to comply with the Mouthful Foods Driver Partnership Agreement, Driver Terms and Conditions, Privacy Policy, Marketplace Policies, and any other applicable policies, procedures, or guidelines published by Mouthful Foods, as amended from time to time.

I understand that providing false, misleading, incomplete, or fraudulent information may result in the rejection of this application, suspension or termination of my Driver account, removal from the Mouthful Foods Platform, and any other action available to Mouthful Foods under applicable law.

I further consent to Mouthful Foods verifying the information contained in this application, including identity, driver's licence, Professional Driving Permit (PDP), vehicle documentation, banking information, criminal record checks (where permitted by law), and any other information reasonably required to assess my eligibility to provide delivery services through the Platform.
